New Patient Information

Vaccinations

Our office does require all patients be vaccinated in accordance with CDC guidelines.  We do accept alternate immunization schedules, however we require that patients start childhood vaccines no later than 6 months of age and be complete no later 2 years of age.

Ages

We treat patients from birth until 21 years.  New patients over 18 years old are not accepted.  

Out of Network Insurances

We are currently in network with most major insurance companies, however

Initial Appointment

Please arrive 15 minutes early for scheduled appointment to complete Medical History form, Benefit Consent Form and review our Office Policy/Procedure. (These forms are all available in the Forms section of our website) 

Please have all previous Vaccination records sent to our office prior to appointment date. 

Please complete Medical Records Release Form if needed, to start the process of having previous medical chart delivered to our office. This can be found in the Forms section of our website. 

Our office requires a 24-hour notice for any canceled or rescheduled appointment, if this notice is not given for a New Patient appointment, this appointment may not be rescheduled.

Due to the high volume of patients, we are not currently accepting new patients, aside from newborn visits.
