| The office staff
ensures that absolute confidentiality of medical records will
be maintained at all times. The entire office staff will ensure
that each medical record is handled in a professional manner
which is designed to prevent loss, misfiling, tampering, alteration,
destruction, and unauthorized or inadvertent disclosure of
any information in the absence of the patients written consent.
Office personnel will maintain the complete confidentiality
of all information pertaining to patient care and will not
divulge any information without the patient's/legal guardian's
written authorization unless the release of such information
is:
- required by law
- necessary for the safety of the patient or any other
individual
Any release of information in response to a court order or
subpoena must be reported to the patient's parent/guardian
in a timely manner. |
