Patient Information Sheet
	This form is required prior to a first visit to our office or if there are any changes on the form of an established patient.  It also includes information on important office policies.

	Treatment Authorization Sheet
	This form allows a parent or guardian to specify additional individuals that can bring their child to our office for treatment.

	Release of Records
	Completing this form allows our office to request or release patient records with another physicians office.

	Immunization-Physical Exam Schedule
	This informational schedule lists all immunizations and physicals recommended by the American Academy of Pediatrics.
