for all plans. We are under contract with many insurance
companies with which we file all claims to the insurance company.
Copays are always due at time of service per your contract
with your insurance company.
If we are not contracted providers with your insurance
company or verification of your insurance has not been made,
you would be required to pay at time of service.
We will assist you as much as possible but it is your responsibility
to know your insurance benefits, co-insurance, and deductibles.
